YOUR OPPORTUNITY


We have an exciting Nurse Care Manager opportunity in our Guymon, OK plant. The Registered Nurse Care Manager is an plant/office-based medical and telephone medical care management position that is responsible for managing the Seaboard Foods medical management program. This individual will develop patients' care plans and provide education, guidance, and support to employees with underlying health conditions and illnesses at the Seaboard Foods processing plant. This individual is responsible for early intervention and assessment, appropriate testing, medical care coordination, recordability review, and communication with appropriate government agencies and internal stakeholders.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Manage the medical system to assist employees with underlying health conditions.
  • Develop the patient's plan of care with the employee's primary medical provider.
  • Facilitate and monitor patient outreach and education to improve employees' overall health and care while the medical treatment and virus management progress for all employees, while working as an intermediary between medical care providers, management, and employees.
  • Conduct an initial and ongoing patient assessment by documenting a medical history record review and developing a patient's plan of care.
  • Act as liaison between the employee and medical providers to ensure all employees receive timely access to quality medical care while promoting optimal clinical outcomes.
  • Monitors patient progress against the plan of care goals, emphasizing patient care needs during transitions and health changes.
  • Assist employees in explaining and reinforcing physicians' instructions and answering other employees' treatment plan questions.
  • Educate patients on disease processes and ways to manage disease progression as independently as possible.
  • Maintain patient privacy by ensuring all medical records, case-specific and provider-specific information, are confidential by HIPAA regulations.
  • Coordinate and schedule outside vendor clinics (Flu vaccinations, vision clinic, mammogram clinic) to enhance employee health and wellness.
  • Manage direct report(s).

QUALIFICATION REQUIREMENTS

Required:



  • Graduate of an accredited nursing program
  • Active RN license in Oklahoma; multi-state licensure preferred
  • Bachelor of Science in Nursing (BSN)
  • BLS certification
  • Minimum 5 years of direct clinical patient care experience
  • Strong nursing assessment skills with the ability to determine and coordinate appropriate care
  • Experience managing chronic condition monitoring
  • Ability to work independently and make sound clinical decisions on-site
  • Skilled in health education and wellness initiatives
  • Maintains professionalism and strict confidentiality in a highly visible role
  • Analytically minded and willing to learn new computer systems


Preferred:



  • Master's degree in nursing
  • Experience in Emergency Department or Case Management
  • Intermediate skills in Microsoft Office (Word, Excel) and COGNOS
  • Spanish-speaking ability highly preferred






SCHEDULE



  • The hours and days of work are established by departmental needs and at the discretion of management, after-hours, weekends, and overnight travel as requested.

  • Primarily an office environment with some need to work in the field.
  • While performing the duties of this job, the employee occasionally works near moving mechanical parts.
