Full Licensure Masters Social Worker

Scope of Work:

In conjunction with the Sr. Director CHE Care Management and Care Coordination teams, coordinate and facilitate education specific to grant funded suicide prevention and treatment training and competency maintenance for evidence based practice. Uses specific age and culture-related physical, intellectual, psychological, and development attributes in the educational plans for staff. Collaborate with a regional and statewide education team. Assist with grant data collection and reporting requirements and collaborate with internal and external stakeholders through meetings and workgroups to strengthen suicide prevention efforts across Corewell Health East for the duration of the grant period ending in September, 2028.

  • Develops/implements the educational plan for the site department of Care Management
  • Site orientation of new staff which is comprehensive and individualized with one-on-one training for three or more weeks.
  • Rounding and telephonic support of staff education needs and problem solving.
  • Ongoing education based on analysis of outcomes from external audits.
  • Education and support for implementation and ongoing use of new electronic medical record system and supplemental ancillary computer systems.
  • Collaborate with other corporate team and other site educators to provide education and support as needed.
  • System Liaison for the education of social/psychosocial needs.
  • Coordinates educational support related to patient initial needs assessment, discharge planning, and addressing all forms of identified abuse/neglect, adoption, substance use disorders, assisting with end of life decisions, providing appropriate referral resources, bereavement support, adjustment counseling, facilitating guardianships and behavioral health concerns (including psychiatric transfers).
  • Works collaboratively with a diverse range of clinical departments and key stakeholder groups to ensure consistent and effective audit readiness across the organization.
  • Analyzes audit recommendations.

Qualifications:

  • Required master's degree in social work.

  • 3 years of relevant experience Must have 3 to 5 years' experience in Care Management Social Work, including discharge planning. Required

  • Related position(s); education experience. Preferred

  • Full Licensure Masters Social Worker (LMSW) - State of Michigan Upon Hire required.
