Overview

Heritage Manor, a DePaul Senior Living is seeking a compassionate and organized Case Manager to support residents and develop and maintain person-centered service plans. The ideal candidate has a combination of education and experience in Human Services, preferably in Assisted Living, along with collaboration skills when working with residents, families, community partners, and regulatory agencies. Responsibilities

RESPONSIBILITIES

The areas of responsibility will include but are not necessarily limited to:

1. Respects and maintains resident rights and confidentiality and demonstrates a caring, positive attitude toward all residents, staff and guests and service providers.

2. Assumes primary responsibility for coordinating and performing services necessary to provide residents with an optimal living situation taking care to enhance their autonomy and socialization skills.

3. Works with residents to encourage healthy connections with family and community as well as develop new interests and form new support systems.

4. Maintains accurate and timely records of case management interventions, appointments, recommendations and follow-up in each resident's file.

5. Provides one-to-one support for residents related to emotional, behavioral, social and emotional concerns.

6. Performs the duties and responsibilities of the Medication Coordinator as assigned.

7. Performs the duties and responsibilities of the Supervisor as assigned.

8. Assists in recruiting, screening and interviewing resident care staff. Assists with the orientation for new resident care employees.

9. Assists in conducting in-service education and training programs for staff on a monthly basis.

10. Attends and participates in staff and team meetings.

11. Makes recommendations for assignments and promotions. Participates in coaching, corrective and disciplinary personnel plans.

12. Assists with the development of all staffing schedules for the Resident Care Department.

13. Completes rounds daily, make recommendations for daily resident care assignments and ensures follow-through of resident care staff. Provides assistance to resident care staff when necessary.

14. Assists the Resident Care Director to ensure that all policies, procedures, and Department of Health regulations are adhered to in the department.

15. Consults with the Resident Care Director as needed concerning resident care and personnel concerns.

16. Assists with the admission, transfer and discharge of residents as needed.

17. Performs other duties as needed or requested by the Director and/or Administrator.

ESSENTIAL FUNCTIONS

Communicate with residents and staff in English.

2. Ability to read and write English.

3. Ability to lift up to 30 pounds unassisted.

4. Ability to visually observe residents, to hear emergency systems and verbal needs expressed by residents.

5. Ability to turn, stoop, bend and stretch in order to assist residents.

6. Ability to stand for prolonged periods.

7. Ability to use equipment competently.

8. Ability to respond to emergencies, i.e. resident choking, evacuation of residents and visitors in the event of fire.



Qualifications

Must be at least 18 years of age.

2. Bachelor's degree with major course work in human services plus one (1) year full-time work experience in a related field; or an Associate's degree with major course work in human services plus three (3) years of full-time work experience in a related field, or a High School diploma or GED plus five (5) years of full-time work experience in a related field

3. Well-developed organizational skills.

4. Ability to work effectively with inspectors, monitoring and licensing agencies.

5. Must have the required annual Tuberculosis (TB) testing and annual Health Assessment completed

6. Must have the required references and criminal record check.

7. Must have a valid New York State driver's license that meets agency's clean driving record policy.
