Overview Wheatfield Commons, a DePaul Senior Living is seeking a compassionate and organized Case Manager to support residents and develop and maintain person-centered service plans. The ideal candidate has a combination of education and experience in Human Services, preferably in Assisted Living/Memory Care, along with collaboration skills when working with residents, families, community partners, and regulatory agencies. Responsibilities * Respects and maintains resident rights and confidentiality and demonstrates a caring, positive attitude toward all residents, staff and guests and service providers. * Assumes primary responsibility for coordinating and performing services necessary to provide socialization skills. * Completes admission paperwork with new residents and resident representative and assists resident with becoming acclimated with the community. * Works with residents to encourage healthy connections with family and community as well as develop new interests and form new support systems. * Maintains accurate and timely records of case management interventions, appointments, recommendations and follow-up in each resident's file. This includes developing of a Person-Centered Service Plan upon admission and reviewed/updated every 6 months and as needed in conjunction with the resident's responsible party, resident's physician and home care agencies. * Provides one-to-one support for residents related to emotional, behavioral, social and emotional concerns. * Works with the Resident Care Director to ensure that the residents obtain/maintain a primary physician of their choice and have access to other services as needed to maintain their optimal health/mental health. * Actively networks and maintains links with other health care and human service organizations. Make appropriate referrals according to residents' needs. * Assist residents in need of alternate living arrangements to make and execute sound transfer and/or discharge plans. * Assists with annual rent increases, coordinating paperwork along with the administrator. Coordinates paperwork when there is a room/rent change. * Conducts annual case management evaluations * Manages and completes Long Term Care Insurance process with resident and resident representative. * Attends and participates in staff and team meetings. * Perform other duties as assigned and is available as needed to ensure that all residents' needs are met. Required Skills & Abilities: * Ability to communicate with residents and staff in English * Ability to read and write English * Ability to lift up to 30 pounds unassisted * Ability to visually observe residents, to hear emergency systems and verbal needs expressed by residents. * Ability to turn, stoop, bend and stretch in order to assist residents. * Ability to stand for prolonged periods. * Ability to use equipment competently. * Ability to respond to emergencies, i.e., resident choking, evacuation of residents and visitors in the event of fire Qualifications * Must be at least 18 years of age. * Bachelor's degree with major course work in Human Services plus one (1) year of full-time work experience in a related field; or an Associate's degree with major course work in Human Services plus three (3) years of full-time work experience in a related field, or a High School diploma or GED plus five (5) years of full-time work experience in a related field. * Well-developed organizational skills. * Ability to work effectively with inspectors, monitoring and licensing agencies. * Must have the required annual Tuberculosis (TB) testing and annual Health Assessment completed. * Must have the required references and criminal record check. * Must have a valid New York State driver's license that meets agency's clean driving record policy. Work Environment Physical Requirements: * Ability to sit, stand, and use a computer for extended periods. Must be able to lift to 20 lbs., communicate effectively, and perform basic office tasks such as typing, filing, and operating standard office equipment.
