What You Need to Know

Responsibilities:

  • Performs functions and patient care coordination consistent with the established plan of care and collaborates with team members to coordinate care/services to achieve optimal outcomes.
  • Communicates to physicians/Non-physician Practitioners (NPPs) and all disciplines involved in patient care regarding changes to the plan of care.
  • Identifies challenges and potential barriers post -discharge to ensure appropriate resources and services are in place prior to discharge from an acute care or community setting. Assists in referring to appropriate community resources and services to optimize continuity of care.
  • Drives optimal patient outcomes by working collaboratively with co-workers, client staff and other health care professionals to ensure continuity of care for the patient and family.
  • Promotes cost effective care by updating and communicating with staff and clinicians regarding strategic partner specific programs and processes in development.
  • Provides continuing education and skills training for both office and clinical staff on current and new strategic partner specific programs and processes.
  • Promotes a safe level of care for the patient through communication with agency staff and physician/NPP and other strategic partners (such as MCO, ACO organization staff); escalates needs for new or changed services to the supervisor.

Qualifications:

* RN, LVN, or Therapist, Therapist Assistant licensed in the state where patient care is delivered.
* Minimum of 5 years patient care experience.
* Working knowledge of Medicare Advantage health plans, ACO's, IPA's, ALF's and LTC.
