SUMMARY: Under general supervision of the Program Director, Medical Director, and in collaboration with the clinical nursing and ancillary staff and providers, the Wound Care Nurse Practitioner (NP) is responsible for ordering, assessing, and providing comprehensive wound care including debridement's to patients attending the Wound Clinic patients on the inpatient units and in the ED. The core Success Factors include: Instill Trust and Value Differences RESPONSIBILITIES: Demonstrates advanced knowledge of diagnostic data related to wound care. Maintains a working environment knowledge of pertinent reimbursement and documentation issues. Assure compliance with standards set by regulatory, and accreditation agencies. Is knowledgeable about planned procedures and communicates with the Program Director to assure that necessary skin substitutes, grafts, dressings and equipment is approved, ordered and available for planned procedures and completes documentation of medical necessity for reimbursement. Stays current with innovative treatments and procedures to promote positive outcomes or provide palliation options given the circumstances. Initiates patient status and participates in the development of a patient care management plan with the registered nurses and provider colleagues as necessary. Contributes to creating and maintaining a work environment that fosters staff satisfaction, employee growth, interdisciplinary collaboration, team building and excellence in customer service across the continuum of care. ; Develops and maintains open and effective communication with nursing staff, physicians and other care providers throughout ambulatory and inpatient settings. ; Collaborates with Program Director for effective scheduling of patient based on acuity. Participates in safety huddles and debriefing sessions as necessary.; Reports on-line occurrence reports (Safety Nets) requiring immediate attention to Program Director and/or Director. Consults with Program Director and/or Director on unusual or difficult technical, personnel and administrative problems. Performs other duties as assigned MINIMUM QUALIFICATIONS: EDUCATION: Graduate of an accredited School of Nursing bachelor's degree in nursing. Certified Wound Nurse ANCC approved, preferred by American Board of Wound Management certification, Certified Wound Specialist (CWS), Wound Care Consultant (WCC), Wound Ostomy Continence Nurse (WOCN). Demonstrate knowledge and skills necessary to provide care to patients with consideration of the aging process, human development stages and cultural patterns in each step of the care process. EXPERIENCE: ;Minimum of two years of experience in an acute health care setting, ambulatory or surgical services and /or in the hospital setting. Previous high volume clinic experience in an outpatient setting preferred. ;Current CPR Certification. ;Wound Care Certification or completes certification within one year.
