Title: Staff Pharmacist - Outpatient Location: Fort Sam Houston, TX 78234 Duration: Long-Term Contract Schedule: 40 hours per week + on-call as needed- Monday-Friday: 08:00 AM - 05:00 PM (0800-1700) On-Call Availability: Sunday-Saturday, 12:01 AM - 11:59 PM (0001-2400) Role & Responsibilities: Works collaboratively with multidisciplinary teams to optimize drug therapy. Receives and reviews prescriptions for the full range of pharmaceuticals to include controlled drugs on the formulary. Determines type of substance, strength, dosage, and quantity to be dispensed in accordance with accepted practice and pharmacy standing operating procedures. Enters prescriptions into outpatient computer system, reviewing profile for allergies, drug/drug or drug/food interactions, and duplicate orders. Coordinates with prescriber to clarify issues or problems with dosing, interactions, or instructions. Obtains the pharmaceutical, prepares package labels, visually inspects the medication in the bottle to be dispensed, and verifies appropriate medication, checks the printed label affixed to the container against the original prescription for correctness. Provides instruction to patients on proper use of medications, and mentors technicians and other pharmacy staff working in/with area of responsibility. Provides medication reconciliation for the patient. May be identified per shift as a Code Responder and will respond to Code Blues during a particular shift. Required Qualifications: Bachelor's degree in pharmacy. Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE). Valid Pharmacist Licensure. A minimum of one (1) year of outpatient pharmacy experience within the last two (2) years post-graduation.
