Job Summary

The Registered Nurse (RN) - Pediatric Practice (Primary Care) is responsible for coordinating and administering professional nursing care for children with special healthcare needs and medical complexity within the primary care pediatric practice. The RN provides comprehensive nursing services including clinical triage, vaccine administration, medication administration, point-of-care testing, office-based clinical procedures, and care coordination.

This role performs telephonic and in-person triage using approved protocols and nursing judgment, provides patient and family education, and supports both primary care and behavioral health populations as needed. The RN collaborates with interdisciplinary teams to ensure safe, timely, and effective care delivery in alignment with the patient-centered medical home model and the mission, vision, and values of Children's Specialized Hospital.

Essential Job Functions

Perform telephonic and in-person triage using professional nursing judgment, ClearTriage protocols, and organizational guidelines.

Assess patient symptoms and determine urgency of care, including recognition of emergent conditions requiring escalation to provider or emergency services.

Obtain and document clinical history, assessment findings, and nursing recommendations using SBAR format in the electronic medical record (EPIC).

Provide evidence-based clinical guidance, education, and recommendations within RN scope of practice.

Schedule same-day, urgent, and follow-up appointments based on clinical assessment and provider guidance.

Provide triage support for both primary care and behavioral health patient needs, including assessment of behavioral concerns and escalation of crisis situations per policy.

Administer vaccines in accordance with physician orders, CDC guidelines, and organizational policies.

Provide patient and family education regarding immunizations, including benefits, risks, and aftercare instructions.

Monitor and respond appropriately to adverse reactions following immunization or medication administration.

Administer prescribed medications safely and accurately in accordance with provider orders and nursing scope of practice.

Perform medication reconciliation and ensure accurate documentation in the medical record.

Support medication refill processes, prior authorizations, and coordination with pharmacies as needed.

Perform and oversee point-of-care testing (POCT) including specimen collection, test execution, quality control, and documentation per organizational and regulatory standards.

Ensure competency, compliance, and quality assurance for all office-based testing procedures.

Perform clinical procedures within scope of practice.

Formal Education and Job-Related Experience

Graduate of an accredited school of nursing. Bachelor's degree preferred.

Experience in pediatric nursing or ambulatory care is preferred.

Certification

American Heart Association BLS Provider Certification required.

License

Current New Jersey RN license.
