Develop and implementindividualized treatment plansin collaboration with patients and care teams. Deliveroutpatient psychotherapy services Engage patients' socialsupportsto improve functioning and outcomes. Coordinate care with community providers and agencies; initiate referrals as needed. Mentor andprovideclinical guidance to mental health practitioners when applicable. Develop and maintain individualized treatment and care plans in collaboration with patients and families. Facilitate individual, group, and family therapy sessions aligned with clinical goals. Coordinate care with other providers and agencies; initiate referrals asappropriate. Ensureaccuratedocumentation and compliance with CMS, MN DHS, and MN 245I requirements. Participate in clinical supervision for Mental Health Practitioners when applicable. Required Qualifications* Doctoral/Ph.D. level advanced degree in Psychology or Counseling OR Masters in Social Work MN DHS Mental Health Professional qualifications per MN 245I.04 Licenses/Certifications* MN licensure asLicensed PsychologistorLICSWupon hire. BLS Tier 1 - Basic Life Support - Multisource required within 180 Days Formal Training/Certification in a suicide prevention modality as applicable based on role within 90 Days required Physical Demands Sedentary: Lifting weight up to 10 lbs. occasionally, negligible weight frequently Pay Range Pay Range: $48.29 to $66.81 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
