Resource Specialist

Position Title: Resource Specialist

Work Location: LOWER Region B: Alachua, Flagler, Hernando, Lake, Marion, Putnam, Sumter, Volusia, Citrus

Assignment Duration: 3 months with possible extention

Position Summary:

* coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs. The caseload is 100 members. This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.

Background & Context:

* Describe the performance expectations/metrics for this individual and their team:

* Compliance with care management policies/procedures including outreach and documentation expectations.

* Individuals will be audited on a monthly basis to ensure charts are in compliance.

* Tell me about what their first day looks like:

* The first day will align with training/onboarding. The first day will consist of setting up computer, completing onboarding tasks, reviewing training schedule, and meeting with the training team. Onboarding is 4 weeks.
